Overview

Understanding Hip & Gluteal Pain

Hip pain is often mislabeled. Pain over the side of the hip is usually gluteal tendon and bursal, while groin pain more often comes from the joint itself — and low back problems can refer into both. Sorting that out is the whole point of the first visit.

Common symptoms

  • Groin or deep hip pain with walking and rotation
  • Pain over the outside of the hip, worse lying on that side
  • Stiffness putting on shoes or socks
  • Pain that returns a few weeks after each cortisone shot
  • Limping or shortened stride after activity

Common Causes

  • Hip osteoarthritis
  • Gluteal tendinopathy and trochanteric bursitis
  • Labral injuries and impingement
  • Referred pain from the lumbar spine or sacroiliac joint
  • Hip abductor weakness

How we diagnose it

  • Exam separating joint, tendon, and spine-referred sources
  • Ultrasound of the gluteal tendons and bursa
  • Imaging review for joint space and structural change
  • Guided diagnostic injection to confirm the pain generator

What to Expect

  1. 01

    Source separation

Joint, tendon, or referred from the back — we confirm before treating.

  2. 02

    Guided treatment

Ultrasound or fluoroscopy places the injection into the confirmed structure.

  3. 03

    Abductor strength

Hip stability work, since weakness is what keeps tendon pain recurring.

  4. 04

    Progress check

Function is retested and the plan adjusts to the response.

FAQs

Hip & Gluteal Pain — Questions Patients Ask

Why does my hip pain keep coming back?

Usually because the injection treated the symptom but the tendon was never reloaded properly — or the real source was the spine.

Can PRP help hip arthritis?

It can help pain and function in mild to moderate arthritis. For advanced joint loss we'll be direct about what it can and can't do.

Do you treat gluteal tendinopathy?

Yes — it's one of the more common ultrasound-guided treatments here, and it responds well to precise placement plus loading.

Will I need a hip replacement?

Sometimes it's the right answer. When it isn't yet, we lay out what the non-surgical options realistically offer.
