Overview

Understanding Low Back Pain & Sciatica

Low back pain and sciatica are the most common reasons patients come to Alpine. The pain may sit in the back, wrap into the hip, or run down the leg with numbness and tingling. The plan starts with identifying which structure is actually generating the pain — disc, nerve root, facet joint, or sacroiliac joint — because each one is treated differently.

Common symptoms

  • Pain running from the low back into the buttock, thigh, or calf
  • Numbness, tingling, or a heavy feeling in one leg
  • Pain that worsens with sitting, bending, or coughing
  • Back pain that worsens with standing and extension
  • Weakness in the foot or difficulty with stairs

Common Causes

  • Lumbar disc herniation or protrusion pressing on a nerve root
  • Facet joint arthritis and degenerative change
  • Spinal stenosis narrowing the space around the nerves
  • Sacroiliac joint dysfunction
  • Muscular deconditioning after months of guarded movement

How we diagnose it

  • Full history and a hands-on neurologic and movement exam
  • Independent review of your MRI or X-ray — including films read elsewhere
  • Correlation of imaging findings with your actual symptoms
  • Diagnostic blocks under fluoroscopy when the source is unclear

FAQs

Low Back Pain & Sciatica — Questions Patients Ask

Do I need an MRI before my visit?

Not necessarily. If you already have imaging, bring it — we review it ourselves. If you don't, the exam determines whether imaging changes the plan.

I've been told I need back surgery. Is a second opinion worth it?

Often yes. Second opinions before spine surgery are a core part of this practice, and many patients have non-surgical options that were never laid out for them.

How fast does sciatica improve?

Some patients feel relief within days of a targeted injection; steroid effects can take one to two weeks to peak. Nerve symptoms recover on their own timeline.

Will injections just mask the pain?

A well-placed injection both treats and confirms the diagnosis. It's paired with strength and loading work so the result holds.
