Overview

Understanding Shoulder Pain & Rotator Cuff Injuries

Shoulder pain that wakes you at night or stops you reaching overhead is usually a rotator cuff or bursal problem, sometimes with a labral component. Ultrasound lets us look at the tendon while you move it, which changes what gets treated and how.

Common symptoms

  • Pain lifting the arm overhead or reaching behind the back
  • Night pain and difficulty sleeping on that side
  • Weakness lifting or carrying
  • Clicking or catching with rotation
  • Progressive loss of motion

Common Causes

  • Partial or full rotator cuff tears
  • Subacromial impingement and bursitis
  • Biceps tendinopathy
  • Labral injuries from falls or overhead sport
  • Glenohumeral arthritis

How we diagnose it

  • Movement and strength testing across each cuff tendon
  • Dynamic ultrasound of the tendon while the shoulder moves
  • Review of MRI or prior imaging
  • Guided diagnostic injection to separate joint from bursa

What to Expect

  1. 01

    Dynamic assessment

Ultrasound while you move the shoulder — not a static image from months ago.

  2. 02

    Precise placement

Injection into the tendon, bursa, or joint that the exam actually implicates.

  3. 03

    Graded loading

Cuff and scapular progression so the tendon gets stronger, not just quieter.

  4. 04

    Recheck

We reimage and retest to confirm the tissue is responding.

FAQs

Shoulder Pain & Rotator Cuff Injuries — Questions Patients Ask

Can a rotator cuff tear heal without surgery?

Partial tears frequently improve with precise treatment and correct loading. Large full-thickness tears may need a surgical opinion, and we'll say so.

Why ultrasound instead of MRI?

Both have a role. Ultrasound shows the tendon moving and guides the needle in real time; MRI is better for deeper structural questions.

Is this the same as a cortisone shot?

Cortisone calms inflammation short term. Orthobiologics aim to support the tendon's own repair response.

How many visits will it take?

Usually a diagnostic visit, the procedure, and follow-ups spaced across the rehab progression.
