What does it mean when back pain goes down my leg?
Leg pain from the back usually means a nerve root is involved — but which structure is irritating it changes the treatment entirely.

What This Pattern Usually Means
Pain that travels from the low back into the buttock, thigh, or calf generally means a lumbar nerve root is irritated. That can come from a disc herniation, from narrowing where the nerve exits, or from arthritic overgrowth in the joints beside it.
The distribution matters. Pain past the knee with numbness or tingling in a defined band points more strongly to a nerve root. Pain that stays in the back and buttock more often comes from the facet joints or the sacroiliac joint.
What makes it better and worse matters just as much. Worse with sitting and bending frequently indicates a disc. Worse with standing and walking and better leaning forward suggests stenosis or the facet joints.
