Why does my shoulder hurt more at night?

Night shoulder pain is one of the strongest signals for rotator cuff and bursal problems.

The Short Answer

What This Pattern Usually Means

Lying down removes the muscular support that holds the shoulder joint centered during the day, and lying on the affected side compresses the bursa and cuff directly. That is why a shoulder that only aches during activity can become genuinely disruptive at night.

Progressive loss of motion alongside severe night pain raises the possibility of adhesive capsulitis, which behaves differently and should be treated according to its phase.

Night pain is a useful marker for treatment response. When it resolves first, it usually means the bursa or tendon is settling even before daytime strength returns.

Don't Wait

When To Be Seen Sooner

  • You cannot lift the arm at all after an injury
  • Rapidly progressive stiffness in every direction
  • Fever or redness around the joint
  • Night pain preventing sleep for more than a few weeks

This guide is general education, not a diagnosis. For anything urgent — progressive weakness, or any change in bladder or bowel control — seek emergency care.

How We'd Approach It

One Physician, One Plan

  • A hands-on exam that loads each candidate structure separately
  • Independent review of your existing films, not just the report
  • In-room ultrasound while you move, so tissue behavior is seen live
  • A diagnostic guided block when more than one structure could be responsible
  • Treatment matched to the confirmed source, not a set protocol
  • A written loading program so relief becomes capacity

FAQs

Questions Patients Ask

Will a cortisone shot fix it?

It can calm the bursa. Whether that lasts depends on treating the tendon and the mechanics behind it.

How should I sleep?

Off the affected side, with the arm supported on a pillow in front of you. It is a stopgap, not a treatment.
