Why does my pain come back a few weeks after every cortisone shot?

The Short Answer

What This Pattern Usually Means

Cortisone reduces inflammation. It does not repair degenerated tendon or cartilage, and it does not build the strength that offloads a joint. When relief consistently fades within weeks, one of three things is happening.

First, the injection may not have reached the target — unguided injections in the shoulder, hip, and hand miss more often than most patients realize. Second, the treated structure may not have been the real pain generator; hip pain from the lumbar spine is a common example. Third, and most often, the tissue was never progressively loaded afterward.

The alternative is a confirmed diagnosis, image-guided placement, an orthobiologic where the tissue itself has changed, and a written loading program so the relief is something you keep.
